Background Authorization

Authorization to release information which must be signed by each applicant you are requesting a background check on.

Disclosure

A disclosure that a consumer report may be obtained for employment purposes. This document is required to be provided to the applicant before the background check is procured. 

Pre-Adverse Action Notice

If you intend to take adverse action based off of a background check or consumer report, you are required to provide a pre-adverse action notice to the affected applicant. You must offer a reasonable amount of time for the applicant to respond. The document provided is a template and must be submitted to the applicant on your company letterhead and signed by your company representative. 

Adverse Action Notice

After adverse action, or after you decide not to hire an applicant based off of the information in a background check or consumer report, you are required to provide notification the adverse action has taken place.  The document provided is a template and must be submitted to the applicant on your company letterhead and signed by your company representative.

Fair Credit Reporting Act

The Fair Credit Reporting Act, 15 U.S.C. § 1681 (FCRA) is U.S. Legislation enacted to promote accuracy, fairness, and privacy of consumer information contained in the files of consumer reporting agencies. 

Summary of Your Rights Under the FCRA

This is a summary of the applicant's rights under the FCRA that covers information such as confidentiality, disputes, and obsolete or inaccurate information and must be provided to every applicant. 

State Law Notices

Additional notices provided to applicants detailing their rights under applicable state laws. 

Credit Report Notice

Certain states and jurisdictions restrict the use of credit reports for employment background screening.  This document is subject to change without notice to reflect state and local laws.

California Summary of Your Rights

An additional disclosure summarizing an applicant's rights during the background check process under California State Law.

Article 23-A of the New York Correction Law

The law prohibits the denial of employment to a person convicted of a crime unless there is a "direct relationship" between the crime and the employment or "an unreasonable risk to the property or to the safety or welfare of specific individuals or the general public." This notice must be provided to an applicant upon request of a background check or investigative consumer report. 

Notice of User Responsibilities

This notice specifies a user of background checks or consumer reports' obligation under the FCRA.

Notice of Furnishers Responsbilites

This notice specifies FCRA obligations imposed on data furnishers to a CRA.
